Medication Administration

 

All medications (prescription and non) must be transported to the school in their original packaging or pharmacy bottle by a parent. Students are not allowed to carry medication to school , in school, or on the school bus. Prescription medications must contain the pharmacy label. The first dose of a medication should be given at home, if possible.

Prior to administration of any prescription medication, a parent must complete the following:

1) prescription medication order form (signed by a physician) and a 2) medical information release form

In addition to the above, those students with life threatening food allergies must also submit their physician's food allergy action plan (FAAN), or the school's FAAN found here:

Prior to administration of any non-prescription medication (i.e cough drops, probiotics), a parent must complete the following:

1) over-the-counter medication form and a 2) medical information release form

This does not apply to over-the-counter medications held in stock in the nurse's office (i.e. Tylenol, Motrin) for as needed during the school day. This release will be found on the parent Emergency Information form sent home to every student at the beginning of the school year.
